University Supervisors - Forms and Materials

University Supervisor Forms - all forms are submitted via the Taskstream Direct Response Folio (DRF) within your Taskstream account.  PDFs can be used temporarily if you are unable to get internet access at a school. You can also type responses in any word processing program. Text can later be cut and pasted into the Taskstream program.

PDF copies of Stockton forms are provided below for reference. All forms are completed digitally via the Taskstream DRF system. You can access by logging into your account at www.taskstream.com. If you do not have an account, please contact fielddirector@stockton.edu.

Taskstream Video Tutorials

This video series below provides an overview of how the web-based program, Taskstream, is used as a Stockton University Supervisor. Please consult your Student Teaching Handbook for more details on your role. These videos focus on the use of Taskstream system. As a tip, while watching videos, you can check the description below the Youtube video to find quick access to specific parts of each video with time stamps. Each video, though, is only about 5-8 minutes in length. If you are a new supervisor, you should watch each video in the order presented. If you need a refresher, you should be fine to select a video as needed!

  • Training for Fall 2020 Final Clinical Practice

    This summer, we launched a new content-specific training format. This format allows us to have a focused conversation about the observation and evaluation of student teachers specific to their licensure area. If you were present, you have access to all training materials. If you were not present or are new as a university supervisor, you can do the steps independently. Below are the specifics! 

    1.  You complete an asynchronous review of artifacts & rating based on the content area(s) you supervise. A shared Google drive is provided. Follow the 5 steps! *If you are new and don’t have access to the folder use the request access feature to request it.
    2. Next, we did a group debrief session synchronously via a Zoom meeting. We reviewed the samples and discussed our ratings. If you didn’t participate, that’s ok. You can still access the video of the session (1 hr in length). Below are each of the recordings. **Make sure you do the asynchronous review before viewing videos or the content won’t make sense!

  • Lesson Planning Tips - Lesson plans serve as important evidence of Domain 1. Use these lesson planning tips to ensure your student teacher’s lesson plans are well formatted and thorough.
  • Danielson Words Sample - When writing formal observations and/or commentary, it’s helpful to use “Danielson words.” These can serve as early indicators of a student teacher’s level of performance. These words are drawn directly from the Danielson Framework rubrics used to evaluate student teachers.
  • Post-Observation Conversation Starters for University Supervisors- use these “starters” to ensure conversation after an observation is aligned with edTPA, Danielson Framework, and ambitious instruction techniques.

  • Sample Student Teacher Lesson Plan- Strong Example - This sample lesson illustrates the general length and depth of lesson plans that are turned in by student teachers. Please ensure your student teacher is submitting high quality, well written plans as part of their formal observations.
  • Sample Student Teacher Lesson Plan- Weak Example - This sample lesson illustrates a lesson plan lacking the length and depth expected of student teachers. Use the review of the lesson prior to an observation to address any weaknesses with the plan so that student teachers can revise prior to a formal observation.

  • General Resource
    • Domain 4 guide sheet- This sheet is designed to support the documentation and evaluation associated with Domain 4 for student teachers. Often, supervisors struggle with finding evidence in this area and may select a “N/A” score. We recommend avoiding this. Instead, use the attached guide to find alternate measures for areas a preservice teacher may not have direct responsibilities such as communicating with families.

 Responsibilities:

  1. Follow University Supervisor Guidelines published on this web page.
  2. Receive and provide electronically typed feedback on student lesson plans via Taskstream collaborative program prior to all formal observations.
  3. Meet with student teacher and mentor/cooperating teacher 7 times (1 meet and greet and 6 observations) during the student teaching experience.
  4. Communicate as needed with mentor/cooperating teacher, building administration, student teacher-candidate, and staff in the School of Education.
  5. Provide student teacher feedback at the end of each observation, both orally and in written form using the School of Education forms.
  6. Report information regarding potential student problems to the School of Education office before mid-term.
  7. Document efforts to resolve problems with mentor/cooperating teacher and student teacher before referring to the School of Education office.
  8. Submit progress report, mid-term and final evaluations by the deadlines in the Student Teaching Handbook.
  9. Use your Stockton e-mail for all official electronic correspondence with students, offices of the university, faculty and staff and in compliance with the University’s Information Technology Services Acceptable Use Standards - https://stockton.edu/information-technology/acceptable-use-standards.html
  10. Attend adjunct faculty/university supervisor meetings —usually one per semester or as needed.
  11. Act in compliance with Stockton Univerity’s Code of Ethics - https://www.stockton.edu/diversity/ethics.html
  12. Enter final grades in accordance with the academic University calendar.
  13. Adhere to all applicable University Policies and Procedures.
  14. Enrollment in direct deposit as required by the State of New Jersey for all state employees.

Mentor Teachers - Forms and Materials

Mentor Teachers - All forms for cooperating teachers are housed on Taskstream. Co-ops will be completing "Teacher-Friendly" versions of the formative and summative evaluations which differ from the supervisor versions. Danielson's Framework for teachers (2013 Edition) - Stockton utilizes Danielson's Framework for Teachers, a state mandated teacher evaluation system, for evaluating student teachers.
